Alitretinoin is used for severe chronic hand eczema that has not responded to other treatment. It is taken orally as a capsule for a limited time. In some countries it is used locally on the skin as a gel for lesions caused by the HI virus (symptom of Karposi syndrome). Alitretinoin is a retinoid, a derivative of vitamin A. These play a role in cell metabolism and growth, cell reproduction and the modulation of immune reactions. Alitretinoin interacts with the retinoid receptors (RAR and RXR) and shows an immunomodulatory and anti-inflammatory effect, which is important for skin inflammation, e.g. substances that promote inflammation (cytokines) are downregulated in the skin cells. Alitretinoin is teratogenic, so women must be guaranteed safe contraception. Common undesirable effects include headache, dry skin, lips and eyes, increased blood lipid levels (triglycerides and LDL), changes in the blood count and depression.

Since only alitretinoin was entered without any further substances, no pharmacokinetic interactions can be detected.
The pharmacokinetic parameters of the average population are used as the starting point for calculating the individual changes in exposure due to the interactions.
Alitretinoin has a low oral bioavailability [ F ] of 30%, which is why the maximum plasma level [Cmax] tends to change strongly with an interaction. The terminal half-life [ t12 ] is 9 hours and constant plasma levels [ Css ] are reached after approximately 36 hours. The protein binding [ Pb ] is very strong at 99.1% and the volume of distribution [ Vd ] is small at 14 liters. The metabolism takes place via CYP2C8, CYP2C9 and CYP3A4, among others.
